Constellation Consulting, LLC works in collaboration with the New Mexico Department of Health, Heart Disease & Stroke Prevention Program to convene the Population Health Learning Collaborative (PHLC).
The PHLC will work to prevent and reduce cardiovascular disease and related illness throughout New Mexico, especially among those most impacted. Using a preventative approach, the PHLC prioritizes addressing and improving SDOH for priority populations and improving overall wellbeing.
The PHLC includes organizations across 11 social service areas that address the social determinants of health, including:
- Housing
- Insurance
- Mental Health
- Medication
- Childcare
- Transportation
- Substance Abuse
- Employment
- Literacy
- Language Translation
- Food Assistance
Each year, PHLC members will participate in quarterly gatherings, trainings, or meetings to make connections, learn, and engage with each other. Throughout their engagement in the PHLC, members will gain new resources, share community challenges, and learn best practices to make data driven decisions. PHLC members will increase referrals for their populations, the effects of SDOH will improve, and cardiovascular health will improve throughout New Mexico.
